In consideration of my role with The Way Calvary Chapel, Inc., its affiliates, integrated auxiliaries and supporting organizations, hereinafter “The Way Calvary Chapel” and recognizing the importance of my character being above reproach, I HEREBY CONSENT TO THE FOLLOWING:

Allowing The Way Calvary Chapel to screen me through the Florida Department of Law Enforcement or any other national, state, county, municipal agency, or private firm for the purpose of accessing and reviewing Florida and national criminal history records, driving records, references, current or prior employment as well as any other historical or background records pertaining to me. This consent also authorizes all re-screening as deemed necessary by The Way Calvary Chapel.

I HEREBY WAIVE, RELEASE, AND HOLD HARMLESS FROM LIABILITY The Way Calvary Chapel, its staff, employees, volunteers, and agents with regard to any decision that it makes on my application for involvement with The Way Calvary Chapel based on the information I provide or that is obtained through the criminal history and background screening process.

I CONSENT to a copy of this Consent and Waiver Form being furnished to any reference that I have provided to The Way Calvary Chapel and to any other person, organization, or entity that The Way Calvary Chapel deems necessary in connection with its investigation of my background, character, or qualifications.

I understand that I will be interviewed by a ministry overseer before approval for this ministry. I also understand that I represent Christ and The Way Calvary Chapel in my involvement with this ministry. I further acknowledge that “conduct not becoming of the Gospel” (Phil. 1:27) whether private, public or on social media, could undermine my witness and damage the church’s reputation. I am prepared to temporarily remove myself if I am dealing with an active problem in my life that can compromise my personal testimony or that of The Way Calvary Chapel.
